Antioch Church of Wright City (the “Church”)

Please read this waiver carefully. By typing your full legal name and checking the box below, you are signing this waiver electronically and agreeing to its terms for yourself and for every person you are registering (each a “Participant”).

1. Voluntary participation. I confirm that I and each Participant I am registering am/are voluntarily choosing to take part in the kickball event organized and hosted by Antioch Church of Wright City in Wright City, Missouri (the “Event”).

2. Assumption of risk. I understand that participation in kickball and related activities involves inherent risks, including but not limited to physical contact, running, falling, collisions, contact with equipment or other participants, weather conditions, and the risk of minor to serious bodily injury. I knowingly and voluntarily accept and assume all such risks, both known and unknown.

3. Release of liability. To the fullest extent permitted by law, I release, waive, and discharge Antioch Church of Wright City, its pastors, staff, volunteers, leaders, and representatives from any and all liability, claims, demands, or causes of action for any injury, illness, loss, or damage to person or property that I may suffer arising out of or related to my participation in the Event, whether caused by negligence or otherwise.

4. No responsibility for injuries. I agree that Antioch Church of Wright City is not responsible or liable for any injuries I may sustain while participating in, traveling to, or attending the Event.

5. Medical treatment. I authorize event organizers to seek emergency medical treatment on my behalf if needed, and I accept responsibility for any resulting costs.

6. Fitness to participate. I confirm that I and each Participant I am registering am/are physically able to participate and not aware of any medical condition that would make participation unsafe.

7. Signing for others. If I am registering more than one person, I confirm I have the authority to agree to this waiver on behalf of every Participant I have listed, and this release applies to each of them as if they signed it themselves.

8. Parent / guardian consent for minors. For any Participant under 18 years of age (a “Minor”), I confirm that I am the parent or legal guardian of that Minor, and I give my consent for the Minor to participate in the Event. On the Minor’s behalf, I agree to all terms of this waiver, including the assumption of risk and release of liability above, and I acknowledge that Antioch Church of Wright City is not responsible for any injuries the Minor may sustain.

9. Acknowledgment. I have read this waiver and release of liability, I understand its contents, and I agree to it freely and voluntarily on behalf of myself and everyone I am registering.
